Creating a Publish Form is done automatically by Enterprise and is triggered by clicking on a Publication Channel for the first time after it has been added to a Dossier in Content Station.
For example: When you open a Dossier, add a new Publication Channel and click on that Publication Channel in the list of Channels, it is then that Enterprise creates the Publish Form. See these steps at work in this video:
